Repair Work Order

If you would like to send your medical video equipment for us to evaluate for repair:

From your computer:

1. Go to Repair Work Order Form
2. Fill out form
3. Save and Email to info@lightsourcevideorepair.com
   or Print and Send in box with equipment

From your Iphone/Ipad:

1. Go to Repair Work Order Form
2. Click share button:
3. Go to Create PDF and click (this will allow you to fill in form)
4. Click share button again to email to info@lightsourcevideorepair.com

Once we have received and evaluated your equipment, we will email you a quote of our findings and cost of repair.
